Infection Prevention Practice in the Burns Patient Setting: a sustainable bundle approach

Background
Burns patients are highly susceptible to infection. Infection is attributed as the leading cause of morbidity and death . Many factors can determine infection risk, but the quality of knowledge of infection prevention and environmental cleaning is essential. This project measures the influence of auditing best practice and compliance to Infection Prevention protocols in a state-wide Burns Service.
Method
This project was performed over a three-months in the Burns ward encompassing 9 beds, equipment and environment. Pre-intervention observational audits were performed to measure compliance for:
•Aseptic Technique (AT)
•Hand Hygiene (HH)
•Standard Precautions (SP)
•Transmission Based Precautions (TBP)
•Shared Patient Equipment Cleaning (SPEC)
Fluorescent marker and black light validated cleaning compliance for:
•Environmental Cleaning (EC)
Interventions implemented included timely feedback to staff, promoting guideline awareness, education sessions, staff surveys and identifying barriers to performing these tasks. Simulation training was utilised to educate and demonstrate cleaning practices inclusive of burns bath cleaning. Post-intervention audits were undertaken to measure these interventions.
Results
Overall, 428 pre-intervention audits were captured and 496 audits post-interventions. Results showed a significant improvement in compliance for all areas measured. The greatest improvement for cleaning compliance was the Burns Bathroom from 59% pre-intervention compared to 90% post-intervention. The greatest improvement for the observational audits were for SPEC from 25% compliance pre-intervention to 88% post-intervention.
Conclusion
Through this dedicated project there was substantial overall improvement in compliance for all IP practice on the Burns ward and supported the utility of a dedicated portfolio.
